To initiate this self service facility please complete this form and upload a selfie (photo of yourself holding your ID document or valid Passport) as indicated in the Proof of your Identity section of this form.
For security reasons, only 1 application will be allowed per person.
Sentinel reserves the right, based on their sole discretion, to not issue you with access credentials for our online member portal.
Once we have verified the information and photo submitted, you will receive your User ID and Password by SMS. This will allow you to access your personal account on the Fund’s interactive web-site.
Sentinel may also in future send relative and important information relating only to your participation in the Fund via SMS and e-mail to the detail provided in this form.
