    Scrubs Order form

  • Order request form for the SAAFP scrubs - pants and tops (embroidered with the SAAFP logo) for both males and females. 

    Please note that the next SAAFP order dates with the supplier:

    • 31 March 2024
    • 30 June 2024
    • 31 August 2024
    • 30 November 2024

    Once ordering is complete, you will receive an email with the banking details and cost per unit. Kindly complete payment as requested and provide proof of payment. Our final order to the supplier will be sent on the date as communicated above. All funds must reach us before that ordering date in order to be processed. Please allow for approximately another two weeks after the ordering date to allow for the processing at the supplier and shipping of your order.

  • Included below are concept images of the scrubs and a sizing chart. The below is an indicative guide compared to standard number if sizing. For example: if you are a female and you wear a size 33/34 pants you will wear a size S (small). If you are a male and you wear a size 32/33 pants, you will wear a size M (medium).
